Of everything creatine gets credit for, brain health is the newest claim and the easiest one to oversell.
The logic behind it is sound. The brain, like muscle, depends on the creatine-phosphocreatine energy system to help buffer and regenerate cellular energy. That creates a biologically plausible question: could increasing creatine availability help the brain under circumstances where energy demands are high, or energy availability is compromised?
Possibly. But “biologically plausible” and “proven” are different things, and this is a topic where it's worth holding that line carefully.

How Creatine Works
Creatine is a compound your body makes naturally and also gets from food, particularly meat and seafood. While most of it is stored in skeletal muscle, brain tissue also relies on the creatine-phosphocreatine system to help regenerate ATP, the immediate energy currency cells use to function.
The brain is metabolically expensive to run, and its energy demands spike under conditions like sleep deprivation, stress, or intense cognitive effort. Supplementing with creatine increases how much phosphocreatine is available system-wide, which is the basis for the hypothesis that it could support brain function specifically when energy availability is a limiting factor, not necessarily under ordinary, well-rested conditions.
The Power Case: What the Studies Actually Show
A 2024 systematic review and meta-analysis of 16 randomized controlled trials involving 492 participants found statistically significant improvements in memory, as well as some measures of attention time and information-processing speed. But there was no significant benefit for overall cognitive function or executive function. More importantly, the researchers rated the evidence for memory as moderate certainty while evidence for several other cognitive outcomes remained low certainty.

The scope of a systematic review found a significant memory improvement with creatine supplementation, rated moderate-certainty for memory and low certainty for other cognitive outcomes.
Another meta-analysis found a small overall improvement in memory, with a larger signal among older participants—but heterogeneity was substantial. A newer systematic review focused specifically on older adults concluded that the available evidence suggests possible cognitive benefit, while emphasizing that the number and quality of studies remain limited.
There are also clues that creatine may become more useful when the brain is under metabolic stress. Studies involving sleep deprivation, for example, have reported benefits in selected cognitive and mood outcomes after creatine supplementation.
That is considerably different from saying “creatine makes you smarter.” It doesn't establish that.
REALITY CHECK
The muscle evidence and brain evidence are not equivalent. For resistance training, strength and lean tissue: the case is relatively strong. For memory and cognition: the signal is interesting, biologically plausible and increasingly supported, but still developing. That's exactly the distinction worth preserving when longevity research starts moving faster than the evidence.
Dosing and Safety, Briefly
Creatine monohydrate is the form with by far the largest research base; more expensive formulations haven't been shown to convincingly outperform it. For most healthy adults, 3–5 grams daily is a practical, evidence-based maintenance dose. A loading phase (about 20 grams per day for five to seven days) fills muscle creatine stores faster, but it isn't necessary—3 grams a day gets you to a similar place in about four weeks. Timing relative to workouts matters far less than people assume; consistency is what counts.
Creatine monohydrate is among the most extensively studied dietary supplements, and research in healthy adults at recommended doses has generally not found evidence of kidney damage. One caveat: creatine can affect serum creatinine, a blood marker used to estimate kidney function, so a lab result may look different than expected. That's a reason to make sure your healthcare provider knows you're supplementing, not a sign of harm on its own. People with known kidney disease, significant chronic conditions, pregnancy or breastfeeding, or those on medications affecting kidney function should talk to their healthcare provider before starting.
What Creatine Won't Do
It has not been proven to prevent dementia.
It is not established as a treatment for cognitive decline.
It won't produce a noticeable cognitive effect under ordinary, well-rested conditions the way it does for strength under training load.
Bottom Line
Creatine's brain-health story is genuinely interesting—the mechanism is sound, and the memory findings are real, not fabricated. But this is exactly the kind of finding that gets flattened into an oversimplified headline. The honest version: worth watching closely, not yet worth promising.
